This Notice of Privacy Practices describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
We are committed to maintaining the privacy and security of your protected health information (PHI). PHI includes information about your prescriptions, medical conditions, and any other health-related information we may collect. We are required by law to maintain the privacy of your PHI and to provide you with notice of our legal duties and privacy practices concerning your PHI.
